NHS Initiatives for 2014/15

All patients over 75 will have a named GP

If a patient at Stuart House, who is over 75 years, sees a GP on a regular basis, that GP will be known as the "named GP" on the medical records.

A dedicated phone number will be given to other organisations (Hospital, Care Home) to enable easy access for urgent situations

Stuart House will supply these organisations with a separate telephone number, not the main Reception/Surgery number

2% of vulnerable patients will be identified in anticipation of avoiding Unplanned Admissions.

Stuart House will hold regular meetings with other members of the Health Care Team e.g. Community Nurse Specialist, MacMillan Nurse, to make sure we provide quality care and keep vulnerable patients out of hospital when an Admission can be avoided.

GP Joining the Practice

On September 3rd we have a new GP joining us, her name is Dr Christina Hallam, and she will be working 3 days per week.

Patient Choice Scheme

We are a Pilot site for the new “Patient Choice Scheme.

Other pilot sites include:-

  • Central London (City & Hackney PCT, Tower Hamlets PCT & Westminster PCT)
  • Nottingham City Primary Care Trust
  • Manchester Primary Care Trust
  • Salford Primary Care Trust

This means if you work or visit one of the above areas you may register with a GP there or be seen as a day visitor.

If you register in one of the above areas, you will be de-registered from this Practice, however if you come home to Boston and wish to see a GP in the evening or at the weekend you can still be treated here, you can be seen at the Practice between the hours of 8:00am to 8:00pm Monday, Tuesday & Wednesday and 8:00am to 6:30pm Thursday & Friday. You may also request a visit where it is deemed clinically necessary. Tel 01205 362173

If you need a GP outside normal surgery hours, evenings or weekends, please dial 111 for medical advice. You may be asked to visit the local hospital to see the “out of hours” GP. If your problem is urgent you can dial 999.
