Community Nurses


The Community Nursing team work under Lincolnshire Community Health Services and visit patients at home who are physically unable to leave their homes to attend appointments. The nurses provide care to meet a variety of health needs including wound care, advice regarding continence, medication administration and care for palliative patients at the end of their lives. The team works in partnership with patients and their families by offering health education and are able to refer to other agencies for support. The Community Nurses run clinics based at two of the town surgeries to provide wound care, Leg Ulcer management and removal of sutures for patients who are not housebound and are able to get to a surgery.
